Consider a resolution authorizing the Mayor to execute Quantity Adjustment/Change Order No. 4 with Chasco Constructors for the Kenney Fort Boulevard (Arterial A) Project.

The original contract for construction services was approved by the City Council on June 23, 2011, for the amount of $17,095,753.80 to extend Kenney Fort Boulevard from Joe Dimaggio to Forest Creek Boulevard.
Quantity Adjustment/Change Order No. 1 added John Harris and Susan Hickox as additional insured, and reinforced concrete pipe was replaced with 36" corrugated concrete pipe for the culvert crossing at the request of the Union Pacific Railroad at the cost of $10,519.
Quantity Adjustment/Change Order No. 2 was for quantity adjustments in railroad related work and field relatedwork and field related quantity changes at a cost of $34,007.70.
Quantity Adjustment/Change Order No. 3 was for unforeseen railroad and street light installation expenses at a cost of $540,949.30. Fourteen calendar days were added to the construction contract.
The total cost of Quantity Adjustment/Change Order No. 4 is $114,669.55, and fourteen additional contract days would be added to the construction contract. The quantity adjustments are for revisions in Storm Sewer System 1A and revisions to the Forest Creek Boulevard intersection layout. These would add four calendar days to the contract at a cost of $19,664.55. The change order portion adds revisions to Storm Sewer System 1A and the Forest Creek Boulevard intersection, and joint restraints to the 16" ductile iron pipe at the water line bore. It also includes installation of 24" steel casing with flowable fill as backfill on Kenney Fort Boulevard at a cost of $95,025 and an additional ten days to the construction contract.
